How does the consultation work?

• Fill out the intake form below - you will then receive a questionnaire to fill out

• Once I receive the completed questionnaire, I will reach out to your veterinarian for medical records and approval to move forward

• Upon approval you will be able to book a 30 minute virtual consultation with me. During our zoom call we will discuss...

  • Your goals in regards to nutrition (what ingredients you prefer, feeding only home cooked food, supplementing kibble with home cooked food, etc)

  • My complete nutritional assessment of your pet and how we can optimize your pet's diet

  • And I will answer any questions you have about your pet's nutrition

• After we finish our consultation I will formulate complete and balanced customized recipes within 7-14 business days​

Who is eligible for this service?

• Dogs with a current/up to date annual examination

• Dogs over 12 months of age (small to medium breed dogs) or 18 months of age (large breed dogs)

How much does the package cost?

• There total cost will depend on how many recipes you would like​

• Package options:

  • One recipe: $400

  • Two recipes: $525

  • Three recipes: $625

  • Four recipes: $700

  • Full medical record review and thorough nutritional assessment

  • 30 minute virtual consultation

  • Complete nutrition plan including customized recipes​

  • 2 weeks of follow up support

  • One minor recipe reformulation (if needed)

When will I get my recipe?​

• Before our consultation, you must receive approval from your veterinarian prior to booking. I will email them a form to confirm that your pet has a current exam, has no underlying medical conditions, and that they approve a homemade diet. In certain circumstances, referral to a board-certified veterinary nutritionist may be recommended.

• After our consultation, you will receive the complete recipe and additional documents (calorie content, cooking instructions, etc) within 7 to 14 business days.
